Methylcobalamin vs. Cyanocobalamin

Not all vitamin B12 is created equal. If you have ever compared IV therapy or B12 injection menus around Sedona and the Verde Valley, you have probably seen two very different names on the label — methylcobalamin and cyanocobalamin. They both count as "B12" on paper, but the way your body handles them is meaningfully different, and it's a fair question to ask your nurse which one is going into your arm. ## Quick answer At Hydrate Or DieDrate we use methylcobalamin. It is the active, ready-to-use form of B12, it does not need to be converted by the liver, and it does not carry a cyanide molecule. For most patients that translates into faster subjective energy, cleaner methylation support, and a better safety profile — especially for anyone with MTHFR variants, kidney concerns, or a history of smoking. ## What is cyanocobalamin? Cyanocobalamin is the synthetic form of B12 that most drug stores, multivitamins, and low-cost injection clinics use. It is cheap, extremely shelf-stable, and easy to manufacture. The molecule contains a cyanide group attached to the cobalt atom at the center of the vitamin. Your body has to strip that cyanide off and then convert the leftover cobalamin into an active form (either methylcobalamin or adenosylcobalamin) before your cells can actually use it. For a healthy person that conversion is generally fine. The amount of cyanide released is very small and is filtered out by the kidneys. But it is still an extra metabolic step, and there are a few groups where it is not ideal: - People with kidney disease or reduced renal clearance - Smokers, who already have elevated cyanide exposure from tobacco - Anyone with genetic variants (like MTHFR C677T) that impair methylation - Patients with Leber's hereditary optic neuropathy, where cyanocobalamin is specifically contraindicated ## What is methylcobalamin? Methylcobalamin is one of the two active, "ready to use" forms of B12 that your body actually deploys inside cells. Because it is already methylated, it can plug directly into the methionine cycle — the pathway responsible for producing SAMe, regenerating neurotransmitters like dopamine and serotonin, and clearing homocysteine. Clinically, that matters for: - Energy and mental clarity — methylation drives mitochondrial and neurotransmitter function. - Nerve health — methylcobalamin is the preferred form in most peripheral neuropathy research. - Detox and homocysteine — better methylation means better homocysteine control, which supports cardiovascular health. - MTHFR patients — people whose bodies do not methylate folate efficiently benefit from receiving pre-methylated cofactors. ## What about hydroxocobalamin? You may also see hydroxocobalamin on some menus. It is a natural intermediate form that the body converts fairly easily into methyl- and adenosylcobalamin. It has a longer half-life than cyanocobalamin and is generally well tolerated.
